As women change and get older, their desire to change their breasts may occur as well. As women age, they often desire to downsize their breast implant sizes. In my practice, I have found that women who are entering into their 40's and 50's desire to have their implants made smaller, sometimes one to two cup sizes. Downsizes implants may also reduce heaviness and weight on the back if the patient feels that the implants are too large and are causing back pain and neck strain, etc.
Breast implants can be reduced in size under general anesthesia through a periareolar approach, removing the existing bags and replacing them with new implants. Saline implants can be replaced again with saline or with silicone. These are the choices that a woman has to make as to her desires. Advantages of the saline implants include no silicone calcification, no silicone leaking to the lymph nodes, and no need to do an MRI every three years, mammograms are sufficient. The advantages of silicone implants are they are usually softer at least initially and early on, they have less visibility and rippling of the bags and they have been found in a lot of women to feel softer, and also have a more natural appearance.
It is not unusual for younger women to decide to have larger implants placed. Sometimes women may go from an "A" to a full "D" and one to three years later may decide to go to a full "C." If this be the case, it is important that the patients have enough tissue stretch in order to allow to safely go to a larger size implant.
Only Board Certified Plastic Surgeons have the ability and experience and judgment to determine whether an implant increase size is safe to be placed and that the woman has enough viable tissue and there will not be too much tightness which could increase risk of scar tissue, capsular contracture, infection or dehiscence of the incision.
In general, we use high profile saline and silicone implant with our exchanges in order to give a fuller appearance to the breasts, especially in the upper pole, however, to remain narrower on the sides so that the woman does not look matronly heavy and wide.
